This page is an archive of several different troop 64 and BSA documents. They will help you perform your job in the Troop better and if the description says that you need to print the document, please do.
Click the form name to download it. Some of these forms may take a while to download, please be patient.

| Troop Handbook | Although the Troop Handbook is a large amount of pages, it is strongly recommended for every Scout. If you cannot print a handbook, please let a Troop Leader know and we'll get one for you. The PDF version is recommended. |

| Boy Scout Application Form | This form is self-explanatory; if you are joining Troop 64, you will want the Troop Health Forms as well (below). |
| Adult Leader Application Form | This form is self-explanatory; if you are joining Troop 64, you will want the Troop Health Forms as well (below). |
| Troop Health Forms and Adult Driver Form | Any new Scout or a Scout with expired Health Forms (they expire every year) needs this form. Also, if you are an adult please fill out the driver form. |
| Boy Scout Health Forms- Class III | This form is required if the Scout is due for a troop Physical. (Take this and have your doctor fill it out at your next physical) |

| Troop Meeting Plan Form | This sheet is useful for the SPL and ASPL. |

| Trip Preparation Form | This form needs to be filled out by each Patrol Leader and turned into the SPL 1 meeting before a campout. |
| Summer Camp Forms | Any Scout attending Summer Camp needs this set of forms, and will also need the Troop and Boy Scout Health Forms. |
